• Birth certificate application form

• Certified copy of the American birth certificate

• Passport or Chadian national identity card of the parent(s), or any other identity  document if one of the parents is not Chadian  

• Check or money order of $20 payable to the Embassy of Chad in Washington DC  We do not accept documents sent by e-mail or WhatsApp.  

All documents must be mailed to the Embassy’s address with a pre-paid return envelope.  The processing time for any application is one week.
